Customer Agreement and Liability Release
![Customer releases Sylvano's Grooming Pet Spa, from any and all liabilities, financial, and otherwise, for injuries to Customer, Customer's pet, or any other property of Customer, which arise in any way from services and/or products provided by or as a consequence of Customer's association with Sylvanos' Grooming Pet Spa, including, but not limited to, veterinarian bills. Customer assumes all liabilities, financial and otherwise, for the behavior and health of their pet. In consideration of the services rendered by Sylvano's Grooming Pet Spa. Customer waives any and all claims, actions, or demands of any nature, foreseen or unforeseen, against Sylvano's Grooming Pet Spa,relating to the care, control, health, and/or safety of Customer's pet arising during services performed bySylvano's GroomingPet Spa. It is agreed: I do here by entrust my pet(s) to Sylvano's Grooming Pet Spa, for the purpose of grooming my pet(s). I am aware that all due care will be taken with my pet(s) for the safety of pet and groomer. I am aware that neglect of my pet's coat can be a cause for problems after grooming such as clipper/brush irritation. If my pet is wiggly or doesn't remain still during the grooming procedure accidents can happen such as nicks or irritation from clippers or scissors at no fault of the groomer. Matted Coat:All attempts will be made to salvage a matted coat; however I will allow Sylvano's Grooming Pet Spa, to make the final decision to cut down the coat if dematting can not be tolerated by my pet. If I do not want the coat cut down, I will assume responsibility to demat my pet’s coat and Sylvano's Grooming Pet Spa, will return at a later date to groom my pet: I will incur a $30.00 fuel fee to be paid to Sylvano's Grooming Pet Spa, for this initial visit should I exercise this option. Refusing Service:Customer understands that Sylvano's Grooming Pet Spa, has the right to refuse service to Customer and his/her pet at any time for any reason. Customer understands that if their pet has a history of or repeatedly demonstrates aggression or biting of humans or animals, Sylvano's Grooming Pet Spa, reserves the right to refuse service. Customer also understands that all bites will be reported to the local authorities as required by law. Legal Owner:Customer must be the legal owner of the pet. The pet must not have been exposed to distemper, rabies, or parvovirus within the past thirty (30) days and has been inoculated as indicated by records presented. Pictures:I understand that Sylvano's Grooming Pet Spa, may take pictures of my pet, before and after grooming, for her website, advertising and education. I will receive no compensation monetary or otherwise for use of my pets image as listed here. Flea/Tick Policy:Fleas and tick will NOT be tolerated. Owners are responsible for keeping the pet flea & tick free. Customer acknowledges and accepts that if Sylvano's Grooming Pet Spa, finds mass fleas or ticks, there is a $75 cleaning fee to de-bug the Sylvano's Grooming Pet Spa, before the next client may be serviced. This fee is in addition to the regular groom fee. Cancellation:In the event I need to CANCEL my appointment: I understand that I must notify Sylvano's Grooming Pet Spa, at least 48 hours prior to my scheduled appointment. Leaving a message on the voicemail is fine. I am also aware that should I fail to do so I may incur an additional $30 Fuel Fee, which if incurred will need to be paid before my next groomappointment is scheduled. Home and Property Damage: Sylvano's Grooming Pet Spa, will not be held responsible for damages to my yard, home or property while on the service call for grooming my pet(s). As the owner or caregiver, I authorize Sylvano's Grooming Pet Spa, to perform scheduled grooming appointments while I am away from home or property. In the case where a key is left or given for my home, Sylvano's Grooming Pet Spa, will not be held responsible for any damages or theft to my property. I will sign an invoice/work order each time my pet is groomed as requested, and I will leave a note with my authorization [signature] for grooming and payment for grooming fees should I not be home. Emergency Care:I authorize Sylvano's Grooming Pet Spa, to act as my agent in the event emergency veterinarian services, boarding, care-taking, and/or transportation is necessary and I agree to pay all related costs. Any/all damages, loss or claim shall include but not limited to death, injury, or shock. Said pre-existing conditions shall not be limited to advancedage, extreme nervousness, neurosis, illness, previous injury, skin or coat conditions, or mental conditions. Payment for Services:By Signing Below I authorize Sylvano's Grooming Pet Spa, to charge for services rendered and acknowledge payment is due at time of service. I acknowledge that any rejected funds (bank declines or returned checks) will be subject to a $30.00 processing fee which will be due and payable prior to my being able to schedule my next grooming appointment. If my payment is rejected (i.e bank declined), I acknowledge all further payments will need to be in cash. I understand that Sylvano's Grooming Pet Spa, accepts cash, debit or credit as payment at time of service. By signing below, I authorize Sylvano's Grooming Pet Spa, to charge my credit card on file as listed below if I am not present at time of grooming; or to take the cash enclosed in a sealed envelope addressed to Sylvano's Grooming at my home; if I am not at home during service. I understand that my pets groom will not be completed without this signed release/contract and I will leave this contract for Sylvano's Grooming to retrieve at our scheduled appointment time if I am not at home to give it to the groomer. I will make a copy of this contract if I need one prior to my groom appointment by download off site.
